By default, all enabled items are 'cascaded' on the ShellToys XP flyout menu. However, several items may be moved to the main context menu, to be placed immediately below the ShellToys XP item. Items with a checkmark beside them will be moved to the main menu. Click an item in the list to toggle its checkmark on and off.

Remember that if an item has been disabled on the Add/Remove Tools page, checking or unchecking its box on this page will make no difference.

After making all the changes you need on the pages of this window, click OK to confirm.
